RELEASE AND WAIVER. In consideration of my being permitted to attend an event, organized by Dealmakers Forums LLC, in-person at the specified event venue(s) (the “Event”), I, the Attendee, hereby forever release, waive, discharge, hold harmless and covenant not to sue Dealmakers Forums, and its past, current and future officers, directors, employees, agents, representatives, owners, affiliates and successors and assigns (collectively, “Dealmakers”) of and from any and all damages, losses, claims, causes of action, litigation or demands (including, but not limited to, personal injury, disability and/or death) of any kind or nature whatsoever which may be incurred, directly or indirectly, now or in the future, in any way related to COVID-19 and in connection with my attendance and participation at the Event. I promise not to sue Dealmakers for any of the foregoing.

ASSUMPTION OF RISKS. Specifically relating to the global COVID-19 pandemic, Attendee acknowledges the contagious nature of COVID-19 and that the World Health Organization has classified the COVID-19 outbreak as a pandemic. Attendee is aware and understands that COVID-19 is highly contagious and a dangerous disease, and that contact with the virus that causes COVID-19 may result in significant personal injury, illness, permanent disability and/or death that cannot be eliminated regardless of the care taken to avoid such risks. Attendee further acknowledges and understands that Dealmakers is not responsible in any manner for any risks related to COVID-19 in connection with the Event. By choosing to attend and participate in the Event, Attendee is fully aware of and voluntarily assumes and accepts all risk of loss, personal injury, sickness, death, damage and expense that may arise as a result. Attendee further understands that the risk of becoming exposed to or infected by COVID-19 at the Event may result from the actions, omissions or negligence of others who may attend the Event or their families, colleagues, or others with whom they may have contact.

In addition to all other rules and regulations relating to the Attendee’s attendance at the Event, the Attendee agrees to comply with all COVID-related procedures that may be implemented by Dealmakers in order to protect as much as possible the health and safety of all attendees. Attendee represents and warrants that as of the date of the Event, Attendee to the best of his or knowledge does not have COVID-19 and has not been recently exposed to anyone who has COVID-19.

This COVID-19 Assumption of Risk, Release, and Waiver of Liability Agreement (“Agreement”) shall be binding on my heirs, executors, administrators, successors, and assigns. Attendee expressly agrees that this Agreement is intended to be as broad and inclusive as is permitted by applicable laws, and that if any portion of this Agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect. This Agreement contains the entire understanding of the parties relating to the subject matter, and shall not be altered, modified, amended, waived or supplemented in any manner whatsoever except by a written agreement signed by both parties hereto or their duly authorized representatives.

Attendee has read and understands this Agreement and enters into it voluntarily in consideration of the opportunity to participate and attend the Event. Attendee acknowledges that he or she is giving up legal rights and/or remedies which may be available to Attendee.
